Sherlock is (according to canon, now!) a virgin, and has had no known romantic relationships of any kind.
He is assumed, in his world, to be in a homosexual relationship with John Watson, but they aren't. John is probably the person he cares about the most, but whether or not he feels any romantic attachment to him is entirely left unresolved. He doesn't express his feelings to anyone, basically, but he obviously cares about him.
Had a minor obsession with Irene Adler, and certainly not sexual, and only arguably romantic. More likely simply interested, and then learned very quickly that interest was a complete mistake. That he cares for her is certain, but to what extent isn't.
