In the game- Quentin had two of his fingers broken by Danger and various bruises at the time from their scuffle. HE HAD HIS NOSE BUSTED BY BEING HIT IN THE FACE WITH PURE DIAMOND but Jenny fixed it for him. Quentin has also gotten into various scuffles but nothing serious has happened.
Occasionally in the city he gets really severe headaches because the city is busy with voices so it's sometimes hard to tune them all out to concentrate on his own million thoughts. To block that, he occasionally indulges in drug habits. He's also taken various drugs for the fun of it but isn't a daily abuser. He abuses when he's particularly bored or when he feels like he needs the extra boost such as the headache incidents.
Mentally, Quentin has some insecurity issues and sometimes suffers from depression and loneliness. Though this hasn't be quite as relevant in game as it was at home.
He probably has more injuries in game but I can't remember them all.

But aside from that, he took some drugs in his quest for finding meaning with his life that pretty much wrecked his body and brought him to near death. However, he managed to escape the trauma by having his mind placed in a higher dimension. So, while his body apparently died, his mind remained alive. He's since recovering from that.
Quentin has also been seen to be prone to psychic seizures when a great power interferes with his mental abilities. Specifically the Phoenix heading to Earth.
