Example:
Name: That's the character in question.
Canon: This field almost writes itself.
What is your character doing?: The sorts of shenanigans they are up to this very second.
Are you willing to have your character get hurt?: If Y, how much and in what situation.
Does your character have a job?: If N, would they like one? If Y, are they hiring?
Is your character on a team?: If N, would they like to be?
What kinds of plots are you after?: C&C encompasses the cliches of almost every genre! If you just have a direction you want to take your character, but no specific plots in mind, feel free to list that, too. Mayhap people will brainstorm. These plots needn't happen immediately either, obviously!
Is your character open for pairings?: If there’s anything you ship, you can list them if you want! Feel free to describe your character's quirks or general type.
Powers: List them. Are these powers widely known?
Skills: What are they good at? Fighting, calligraphy, underwater basketweaving, etc? Anything you'd like to work into a plot.
Likes: This also applies to personality traits that they gel with.
Dislikes: Personality traits apply here, too.
Anything else?: Free space for all your tl;dr needs. NOT JUST AN INVITATION TO POST A FUNNY GIF.
KYOUKO SAKURA
Canon: Puella Magi Madoka Magica
What is your character doing?: Lurking, being a jerk, and stealing things.
Are you willing to have your character get hurt?: Sure.
Does your character have a job?: Haha. No.
Is your character on a team?: Hahaha. No.
What kinds of plots are you after?: I don't really have a preference! Things involving planning and intrigue would probably not be Kyouko's strong suit, but she's willing to let other people do that if she can yell at them enough.
Is your character open for pairings?: Not for anything functional, but awkward crushes and romantic tension are fine.
Powers: Being a magical girl (i.e. transforming into a colorful outfit and summoning spears out of nowhere, being stronger and faster than she should be). Creating illusions, although that one is currently inactive due to angst.
Skills: Talking tougher than she is. Dance Dance Revolution.
Likes: Food. Also food. And food as well.
Dislikes: Altruism. People who waste food. Things that remind her of her old self--that is to say, mostly selfless heroics, but there's also some indication that general vaguely Christian religious sentiment and loving family were important to her on some level (especially the latter), so while it didn't really come up a lot in canon, she'd probably have some reaction to the subjects if they did come up here.
Anything else?: Not that I can think of.

or on a more serious note karkat will be joining the phantasm in the very near future and

and it's going to end in him causing the death of at least one of his native comrades
if you want to get kyouko involved with him on that front
