Summer Gift Exchange! Give me your information!

Hi! A while ago I did a little survey concerning a summer gift exchange. I have been kinda slow on the uptake with that, but here is a post in which everybody who wants to participate can give me their information. I will then draw pairs at random and PM everybody with their partner's preferences.

A few people expressed that they'd like to mail items, so included is a section about your preferences concerning that. I'll only ask for your mailing address if you indicate that you're willing to give it and you are paired with somebody that must mail items.

I might have picked a bad time for this, because I know a lot of people are on hiatus! For that, I apologize. Spread the word if you know anybody who might want to participate but might miss this post.

I'll PM everybody with their partner's information on June 8th. There will be a post in GDS some time in the first week of August for the big reveal. This gives everybody about two months. Comments to this post will be screened for maximum surpisitude. Without further ado:
