• User ID / Handle / Screen name (which is what you'll use for logging in)

• Password (confirmation)
  
• Nickname (not used for logging in)
  
• Website
  
  The following items have a drop-down menu to the right. The menu options are "Public / something that is likely friendlocked / Private"
  
  
• Gender
  
• Blood Type (hi japan)
  
• Address (the list starts with Tokyo then lists a bunch of other areas of Japan; the very last option is "Overseas")
  
• Year of Birth
  
• Date of birth (month, date)
  
• Occupation (with such options as Professional, Series Creator, Elementary Student, and Ten-Moth)
  
• About Me (bio)

Then there is another button. I think that is the end of the registration process.
