if you asked him, and you were someone he was at least fairly open with, he would only name bruce and talia. talia because she's the only person he's ever interacted with as a family unit--they moved around together, and lived together, and he calls her mama or mother or what have you. he always imagined bruce being part of their unit, and will always call him father, so he counts as well. however, he feels duty-bound to continue considering bruce family, even when he's angry with him, and after talia disowns him he still considers her family, too. basically it's only his parents and even then it's complicated.

he calls ra's grandfather, but this more a bloodline thing. in fact blood is pretty much how damian defines family, even when they are estranged/he hates them. he will never call the other batkids by familial terms (fandom, dick will never be 'big bro,' let it go), but in reality they and alfred are more family to damian than even his parents. he will just never acknowledge this except subconsciously.
